The 4th picture just shows he is skinny, you can't discern whether or not he has lean muscle under the clothing or not.Here is a complete story for those interested. Look at the pictures in sequence. Looks great in 2021...Looks great and about the same build as me in 2022 boat photo....starting to look scary in picture 3, and for a man in his 30's at his height looks malnourished in picture 4.

He'll live longer as a result of this weight loss.So you think that someone who is in the mid to late 30's, trained extremely heavy for more than half of those years and lost probably 45% of their total bodyweight, has what looks to be an oversized head, feet, next to no muscle mass on their arms, pencil thin legs, droopy shoulders, isn't in a state of "unhealthiness" or very close to being there?
