I have not read the entire thread so I apologize if this has been said before.
If I'm the head coach, or the department manager, or the CEO, I feel that success is my responsibility. Sure, some things will happen along the way that are out of my control, but it is my job to constantly assess the risks and mitigate them with the resources at my disposal. This involves planning, constant assessment, and revising the plan as needed.
The resources available to Mac at the University of Florida are way above average in my opinion. For the team to perform this way, especially on offense, leaves no excuses for Mac. He is simply not doing his job as well as his competition. I do not know how anyone could say that it is not the coach. The coach is ultimately accountable. If his players do not perform, it is his fault that they are not prepared or motivated. These boys are young. It is even the head coach's job to make sure they keep their noses clean and stay out of trouble. Sure, there will always be some bad apples. But suspending 10 players for the first game has to fall on the head coach at least to some degree.
If the program is successful, the head coach has to get a lot of credit. Usually that does not happen by accident. If they fail, the head coach has to be accountable.
